In etrieve central, select Data Info Request or use this link: https://iwutestcentral.etrieve.cloud/#/form/212
Fill out pertinent information.
Submit the form.
Verify that it was received and approve it (email notifications will not be turned on).

The form should be in the Advancement area in etrieve Content with a doc type of ADVM Data Info Request.
